in bubble wrap and place in a box large enough to fill in with styro-foam
peanuts (minimum of 2" on all sides of unit). Place a copy of this form
inside the box. Put a label on the outside to the above address. Ship by
UPS, Mailboxes etc., or US Mail. Also, please insure for equipment
